Music Camp Registration

Eligibility

          

This camp is open to children who have completed at least the 1st grade through the 6th grade.

Terms

     
I hereby release First Baptist Church Springfield (FBC) from all liabilities should my child be injured. I understand that my child must be signed in and out each day by an adult, and that my child must be picked up on time at the conclusion of camp. I have provided the camp staff with an emergency contact number where I or another adult may be reached. Should my child be injured I give FBC permission to seek treatment for my child. Additionally, I authorize FBC to use pictures and or video clips that include by child for the promotion of future events.
